Hello all! I seen several references to surveys used for post Death Cafe evaulation, but I cannot seem to find any official Death Cafe survey/evaluation forms on the site. Any guidance would be appreciated, thanks!

What do we do with the evaluations?
Obviously the evaluations are usual information for the facilitators but are they meant to be given to the organization as well?

Thanks Jon! I located it yesterday in the practitioners guide, it had somehow eluded me before that. In the meantime I have sent it out and am already receiving response. I appreciate your reply :)
